Case Management

At BHN, we work with clients to understand their specific needs and support them in building a personalized treatment plan across a team of providers.  As a Licensed Master Social Worker (LMSW) and Certified Alcohol and Substance Abuse Counselor (CASAC), I utilize both my clinical skills and years of experience to secure a range of services for the individuals and families I work with.  After identifying the presenting issues, we work with clients to find resources that best meet their specific needs.  We do this by providing them with services we offer, as well as hand-selected referrals to vetted programs, individual providers and specialized schools.  In addition, we assist in acquiring and/or managing insurance coverage and a variety of benefits to meet our clients’ needs.

• Inpatient and outpatient treatment programs
• Individual providers for psychotherapy and medication management
• Medical and other providers for general healthcare and wellness needs
• Transitional / sober living
• Facilitate interventions
• Recovery and life coaching
• Aftercare programs to facilitate seamless transition out of hospitals and treatment centers, coordinating with the client’s current therapeutic team

Educational Resources
We guide clients and connect them with the best specialized schools and support resources to meet their specific learning needs, including:

• Evaluate and select specialized schools & therapeutic learning environments
• Selecting educational testing practitioners
• Support in acquiring accommodations for learning challenges in both mainstream and specialized learning environments
• Connect with tutors specializing in learning challenges
• Support in development of effective study habits based on learning style

Health Insurance Coverage

• Assisting in filing claims for out of network coverage reimbursement
• Acquiring private insurance coverage and/or a marketplace insurance plan
• Understanding your plan, coverage and explanation of benefits (EOB)
• Advocating for coverage eligibility and reimbursement of services
• Acquiring prior authorizations for medications not on a plan’s formulary list
